"Physician's Certification – There is a NEW form for 2007 that MUST be completed by your physician. This form will be handed out at registration. Please keep in mind that no participant will be able to step on the field August 1st without this form filled out properly.

2 Copies of Year End Report Card : Pop Warner requires each participant to demonstrate satisfactory academic achievement as evidenced by their report card. Two CLEAR Copies are required – One for the participant's registration record, one to submit to the Jersey Shore Conference and one for Pop Warner National. (NOTE - Copies of Report Cards are not required for the Flag Program.)

Any other paperwork that was not handed in at registration MUST be brought to the Rec Center on this day in order for your son/daughter to be rostered onto a team list.
Providing this information for each participant prior to the start of the season is mandatory, Participants will not be eligible to play/cheer or begin practice in the 2007 season until the required documentation is received. There are absolutely NO EXCEPTIONS to this policy.
- Original Birth Certificate and One Copy – We must see the original birth certificate but will immediately return the document to you. We will keep the copy for our records. Returning players have their birth certificated on file.
- Proof of Medical Insurance – Please know the name and policy number of your insurance carrier, along with the participant's primary physician name and phone number. You will need this information to complete the Registration Record.
Wallet Size Photo of the Participant (2in x 2in) – Prior to each Football Game or Cheer Competition, each participant is certified. The photograph is a required part of each participant's Registration Record.

Pop Warner is a national organization with stringent guidelines for registration. The Pop Warner national organization, along with the Jersey Shore Conference, requires very strict proof of each participant's age and weight, as well as a Doctor's Note clearing the child for athletic competition in Football or Cheer. In addition, Pop Warner requires every player/cheerleader to maintain a satisfactory level of academic achievement in order to participate.
